What is this medicine?

RAPACAN (Sirolimus) is an immunosuppressant. It is primarily used to prevent organ rejection in patients who have received a kidney transplant. It works by weakening the body’s immune system to help it accept the transplanted organ. It may also be used to treat certain rare lung diseases and other conditions as determined by a doctor.


What should I tell my health care provider before I take this medicine?

They need to know if you have any of these conditions:

  • liver disease

  • high cholesterol or triglycerides

  • infections or a weakened immune system

  • history of cancer (especially skin cancer)

  • lung or breathing problems

  • intolerance to lactose or galactose

  • an unusual or allergic reaction to sirolimus, tacrolimus, cyclosporine, or any other medications, foods, dyes, or preservatives

  • if you are pregnant or trying to get pregnant

  • if you are breast-feeding


How should I use this medicine?

Take this medicine by mouth, exactly as directed by your doctor. It is usually taken once daily, at the same time each day, either consistently with food or without food.

Swallow the tablet whole with water. Do not crush, chew, or split the tablet.

Your dose may be adjusted based on blood tests.

Overdosage: If you think you have taken too much of this medicine, contact a poison control center or emergency room immediately.


What if I miss a dose?

If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose. Do not take double or extra doses.


What may interact with this medicine?

  • cyclosporine (use with caution or avoid)

  • antifungal drugs like ketoconazole, fluconazole, itraconazole

  • antibiotics such as clarithromycin, erythromycin, rifampin, rifabutin

  • antivirals such as ritonavir or other HIV protease inhibitors

  • anti-seizure medications like carbamazepine, phenytoin

  • grapefruit juice or grapefruit (avoid entirely)

  • St. John’s Wort (avoid)

  • cholesterol-lowering drugs (statins)

What should I watch for while using this medicine?

  • You will need regular blood tests to monitor drug levels and kidney function.

  • Avoid live vaccines while on this medicine.

  • Limit sun exposure and use sunscreen—sirolimus may increase your risk of skin cancer.

  • Inform your doctor if you develop signs of infection, such as fever, chills, sore throat, or cough.

  • Inform your doctor of any new breathing issues—this medicine can cause rare but serious lung problems.

  • Women should avoid becoming pregnant while on this medicine and for at least 12 weeks after stopping it. Use effective contraception.

  • Avoid grapefruit or grapefruit juice as it may increase the drug level in your blood.


What side effects may I notice from this medicine?

Side effects that you should report to your doctor or health care professional immediately:

  • signs of infection (fever, chills, sore throat)

  • trouble breathing or persistent cough

  • unusual bruising or bleeding

  • swelling in legs, hands, or feet

  • mouth sores or ulcers

  • chest pain

  • signs of allergic reaction (rash, itching, swelling, trouble breathing)

  • new or changing skin lesions

Side effects that usually do not require medical attention (report if persistent or bothersome):

  • headache

  • nausea or vomiting

  • diarrhea or constipation

  • joint pain

  • acne or rash

  • increased cholesterol or triglycerides

  • difficulty sleeping

Where should I keep my medicine?

  • Keep out of the reach of children.

  • Store at room temperature between 20 and 25 degrees C (68 and 77 degrees F).
